1. Insufficient Backup Planning
By failing to backup your databases fairly often you are opening yourself up for big problems. Backup planning should involve regular backups, ability to gather and recover backed up data, and use of locations other than database server for storage purposes. Be sure to backup early and often. You will never want to come across a scenario where you don't have the backed up data.
2. Using an Ad Hoc Query
Stored procedures on the application level should be the only way a user can access the database. Permissions like INSERT, UPDATE, SELECT, and DELETE are usable when users are writing their on code with an Ad Hoc query. 3. Not Enough Testing
